Job Description: The Social Worker I position is responsible for the coordination and delivery of activities related to the treatment of a defined caseload or population group. This position is responsible for social assessments and co-occurring disorder identification upon admission. This position provides psycho-educational and co-occurring disorder (COD) groups and attends assigned therapy groups. This position is responsible for the completion of designated continuity of care tasks related to discharge of residents. This position is responsible for various utilization management activities. This position provides consultation and feedback to the Treatment Team regarding assessment, treatment and diagnosis of defined caseload or population group to ensure treatment needs are met. This position acts independently within the parameters of a job description and coordinates activities with the Admissions Department, Treatment Team Coordinators, assigned treatment teams and Aftercare Coordinator. This position reports to the Assistant Director of Treatment Services. Job Responsibilities: Develop and update COD-E and psychological education curriculum. Completes activities related to discharging residents to ensure a smooth transition to the community or alternative placement. Identifies client needs and risks and communicates them to the Treatment Team. Develops the case formulation document. Schedules case formulation meetings within seven business days of admission with the Treatment Team and other key members of the service delivery team. Attends the admission interview and orients the youth and legally authorized representative to the parent handbook and the campus behavior system. Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. Maintains current training status on all training scheduled and assigned to ensure competent practice. Identifies resident and family education needs and barriers to learning. Delivers psychological-educational group services and co-occurring disorder (COD) education group services on specified topics. Completes the social assessment and COD identification, according to agency policy. Obtains consents as defined by facility policy. Responsible for performing various utilization management activities including: review of patient care activities to evaluate the clinical necessity of continued treatment; treatment teams discharge planning; and medical necessity and timeliness of provision of professional and support services. Knowledge of child and adolescent mental illness. Knowledge of a biopsychosocial approach to social work practice. Skill in interviewing and gathering information. Knowledge of Joint Commission Standards for Behavioral Health Care Organizations. Knowledge of community resources/agencies. Knowledge of Social Assessment formulation. Skill in effective communication. Skill in establishing work priorities and managing multiple tasks. Ability to work effectively as a member of a multidisciplinary team. Skill in reading, interpretation and application of agency/facility policy. Knowledge of the treatment planning process. Qualification & Experience: Licensed by the Texas State Board of Social Worker Examiners as a Licensed Baccalaureate Social Worker (LBSW), Licensed Master Social Worker (LMSW), or Licensed Clinical Social Worker (LCSW). Provisional license accepted from out-of-state applicants (may obtain licensure during the probationary period of employment). Experience in a mental health and/or residential treatment setting is preferred.
