Job Description: Reporting jointly to the Sr. Director, UHN Connected Care and the Director, NORC Innovation Centre, the Integrated Care Lead will be responsible for leading the development and implementation of a new integrated system of care in NORC buildings enabled by digital health. The successful candidate will be responsible for aligning multiple internal and external stakeholders to deliver in-person and digital health and wellness services, as well as engaging different levels of leadership across the healthcare sector in its design and operation. Job Responsibilities: Manages procurement of goods and services, vendor contract, relationships and delivery Provides input and recommendations to the policy and research programs at the NORC Centre Regularly monitors stakeholder engagement and reports on implementation progress, risks and issues to Director, NORC Innovation Centre and Director, UHN Connected Care on a regular basis and identifies strategies to minimize/ resolve Works closely with internal and external stakeholders to design, implement and evaluate service models, including a strong focus on systems level change and providing backbone support including change and project management Develops and evolves strategic partnerships across sectors to deliver services and programming Manages NORC site coordinators and oversees community-engagement at NORC locations Maintains program budget and collaborates regularly with Operations Director; takes remedial action to attain budget targets in consultation with the Director, UHN Connected Care and Director, NORC Innovation Centre Assumes overall end-to-end ownership of multiple NORC initiatives throughout the project life cycle, and ensures milestone and on-budget completion Develops, implements, manages and reviews short and long-term plans, and assesses alignment with mission and goals of the NORC Centre Leads the development and transition to operations of a UHN NORC model at early adopter sites, including the implementation of in-person and virtual health services and social/recreational programming Regularly interfaces with project stakeholders at various levels to build and maintain strong relationships Job Requirements: Strong tactical and critical / strategic thinking skills Experience leading health care implementation projects, preferably in an integrated care setting Demonstrated ability to utilize both project and change management methodologies effectively Ability to independently manage multiple projects and prioritize competing needs Proven ability to adapt to a fast-paced, dynamic environment Ability to communicate in a clear, concise manner both written and verbal Excellent interpersonal and stakeholder relationship skills Able to communicate effectively with senior management from both internal and external healthcare organizations, and project stakeholders Highly skilled at anticipating and analyzing risks and issues, exploring alternatives, and developing appropriate recommendations with project team and subject matter experts Ability to perform in multiple roles and contribute to project, operational or strategic initiatives Highly competent computer skills, including Microsoft Office, Google suite, Zoom, and Outlook applications Excellent at team-building, listening, coaching, group facilitation and conflict resolution skills Demonstrated track record of successfully delivering complex, multi-stakeholder projects in a public-sector environment Job Details: Company: University Health Network Vacancy Type: Full Time Job Location: Toronto, ON, CA Application Deadline: N/A Apply Here careers-trivia.net
